GoldLeaf HomeHealth is seeking RN's to service Southwest Denver Metro areas, including Lakewood, Englewood, Littleton, Highlands Ranch and more!  Our visit rates range from $65/visit to $110/open.  Part-time & Full-time positions available.  Our home health agency is dedicated to providing our Registered Nurses a safe and rewarding work environment, as well as flexible schedules and competitive wages. Position Requirements:

  • Available at least 3-5 days a week
  • Active RN/LPN license in Colorado, free of Discipline or Board Actions
  • Must be able to pass a background check, motor vehicle record check, and drug screen
  • Must have a reliable, safe vehicle and be willing to travel to client's homes in the Denver/South Denver Metro area.
  • Familiarity with OASIS strongly preferred
  • CPR and First Aid Certified
  • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing
  • Current driver’s license and reliable transportation 
  • Ability to work a flexible schedule and travel locally

Responsibilities:

  • Visit patients in their homes for one-on-one visits.
  • Complete initial and ongoing assessments to identify the physical, psycho-social, and environmental needs of home health patients. 
  • Assess patients’ conditions and chart observations, and perform evaluation tasks including vital signs and medication review.
  • Administer medication as prescribed by the patient’s physician.  
  • Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life.
  • Provide effective communication to patients/clients, their family members, team members, and other health care professionals.
